A logistics firm from Derby has been prosecuted by the Health and Safety Executive (HSE) following an accident in which a woman lost part of her leg.
NYK Logistics, which distributes parts for Toyota, was held responsible for the accident in 2006.
Lisa Ramos was hit by a forklift truck which knocked her to the floor and ran over her left leg.
This caused multiple injuries and Ms Ramos had to have her lower leg amputated in hospital.
Melvin Sandell, prosecuting on behalf of the Health and Safety Executive, told a hearing at Derby crown court: "The force of the impact [on Ms Ramos] was so serious it resulted in muscles tearing away from her bone. The HSE view this accident as very serious and the decision to prosecute has not been taken lightly."
All employers are obliged to provide a safe working environment for workers and the HSE will prosecute where appropriate any cases in which this requirement has not been fulfilled.

A company has been prosecuted by the Health and Safety Executive (HSE) following an accident in June 2007.
An unnamed worker at Goodyear Dunlop in Birmingham was inspecting tyre casings when a pallet fell off a forklift truck passing by.
They fell onto the worker, fracturing two of his ribs, Abeceder reports.
Goodyear Dunlop pleaded guilty to breaching the Health and Safety at Work Act 1974 and was ordered to pay a fine of ?13,500 plus costs of £2,888.
The firm also had to pay ?4,000 to the injured worker.
HSE inspector Amarjit Kalay said: "The injuries inflicted could have been significantly worse, or even fatal. Operators of lift trucks must ensure that they operate them in accordance with training and employers must ensure that they are used safely."
All employers are obliged by law to ensure the safety of their employees under a duty of care.
They must provide a safe workplace, safe systems of work, safe equipment and safe fellow employees. The construction industry also has its own safety guidelines for specific circumstances.

An investigation has been launched in the procedures used at the Nestle chocolate factory after a 47-year-old woman broke both her legs in an accident recently.
Nestle has made clear its intention to look into how someone working for them through an agency was able suffer such a serious accident while driving a forklift truck.
The accident is the second of its kind to occur at the site in Wigginton in the past 12 months and one employee told the York Press that they were shocked that two such incidents could be allowed to happen in such a short space of time.
In addition to Nestle’s own investigation, the Health and Safety Executive has indicated its intention to play a part in finding out what happened to the female forklift driver who is reported to have needed part of her leg amputating.
Last month, the BBC reported that a man working at a warehouse in Warwickshire needed the help of emergency services after he was trapped between a forklift truck and a pile of pallets.
